Under normal circumstances, Lifeline/Affordable Connectivity Program free government cell phones are limited to one-per-household. However, there are exceptions. That’s where the Lifeline Household Worksheet comes in.
There are two primary ways you can qualify for the exemption and be permitted to have multiple Lifeline/ACP phones in the same household. First, if you live in a group home such as a senior citizens retirement home you will probably qualify. Second, if you are family members who share the same home, but who live independently from each other (for example, each buying your own food, each paying a percentage of the rent, etc), you will probably qualify.
The Lifeline Household Worksheet helps to determine if more than one person in your household qualifies for a Lifeline/ACP benefit. It asks you just a few simple, basic questions regarding your household. You may be required to submit a completed copy of the worksheet in order to legally enroll in the Lifeline/ACP program, or to complete your Annual Recertification.
